This document is in Adobe PDF format, and may be completed in one of two ways. The first and most preferred method is to complete the form by typing in your answers on the computer, and then print the completed form and mail it to the Foundation. Alternatively, you may also print the form blank, neatly handwrite your answers, and then mail it to the Foundation.
